SoGoSurvey http://www.sogosurvey.com/quiz-maker.aspx is worth a mention. It has some really cool themes under its Look and Feel section which makes the quizzes appealing and fun. Also, they are pretty simple to make.

That was really good learning about quizzes and how to make them. This is a really good website and I can't wait to make a quiz of my own. I loved the part where it said it might change over time. That is right. In two years times will be different and we need to change parts of the quiz. I also loved the pictures. How it was showing step by step with pictures also. I would give this website 5 stars. I will compare it with other websites to see how it is different from each other.
